Our Policies

Developmental Disabilities Waiver (DD)

The DD Waiver provides access to 24-hour, seven days a week supervision through Residential Habilitation and Day Habilitation Services and Supports. Service providers are responsible for supporting individuals in services with locating appropriate living arrangements. Living arrangements may include host home settings with 1–2 persons, individualized settings of 1–3 persons, and group settings of 4–8 persons. The DD Waiver also supports participants who live in their own home or who live with and/or receive services from family members.

Home And Community-Based Services Waiver For Persons With Developmental Disabilities (DD)

The DD Waiver provides access to 24-hour, seven days a week supervision through Residential Habilitation and Day Habilitation Services and Supports. Service providers are responsible for supporting individuals in services with locating appropriate living arrangements. Living arrangements may include host home settings with 1–2 persons, individualized settings of 1–3 persons, and group settings of 4–8 persons. The DD Waiver also supports participants who live in their own home or who live with and/or receive services from family members.

 

Individuals who receive services through the DD Waiver are also eligible for all Health First Colorado covered services, as long as those services are not duplicative.

Who Qualifies?

To enroll in the DD Waiver, an individual must meet financial and program criteria:

Level Of Care

An Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF-IID) level of care, as determined by the functional needs assessment (as defined in 42 CFR 440.150).

Eligibility Group

  • The individual must be determined to have a developmental disability.

  • The individual must be 18 years or older.

  • The individual must require access to services and supports 24 hours a day.

Financial Requirements

  • Monthly income must be less than three times the current Federal Supplemental Security Income (SSI) limit. (Refer to the SSI website for current limits.)

  • Countable resources must be less than $2,000 for a single individual, or less than $3,000 for a married individual.

  • If the individual does not meet these financial requirements, they may still be eligible through the Health First Colorado Buy-In Program for Working Adults with Disabilities.

Privacy Policy

Care Foundation Services (“Care Foundation,” “we,” “us,” or “our”) respects your privacy and is committed to protecting it. This Privacy Policy explains how we collect, use, disclose, and protect information when you visit our website, contact us, or use our services.

  • Inquiry details (messages you submit, scheduling requests, service questions)

  • Intake or service-related information you provide (for example, preferences, support needs, emergency contacts)

  • Any other information you choose to share with us

  • IP address, browser type, device identifiers, operating system

  • Pages viewed, time spent, clicks, referring/exit pages

  • Approximate location (derived from IP address)

We may receive information from:

  • Case management agencies or referral sources (where permitted/authorized)

  • Service providers we use to operate the Site (analytics, hosting, forms)

  • Public sources (limited, as needed to communicate with you)

3) How We Use Information

We use information to:

  • Respond to inquiries and provide customer support

  • Provide and administer services, scheduling, and communications

  • Personalize and improve our Site and service experience

  • Maintain safety, prevent fraud, and secure our systems

  • Comply with legal obligations and enforce our policies

  • Conduct internal operations (training, quality assurance, and oversight)

4) How We Share Information

We may share information in the following situations:
 

A. Service Providers (Processors)

We may share information with vendors that help us operate our business (for example, website hosting, analytics, email systems). These providers are expected to protect the information and use it only to perform services for us.
 

B. Legal, Safety, And Compliance

We may disclose information when required by law or when we believe disclosure is necessary to protect rights, safety, and security, or to respond to lawful requests.
 

C. Business Transfers

If we are involved in a merger, acquisition, financing, reorganization, or sale of assets, information may be transferred as part of that transaction.
 

D. With Your Direction/Consent

We may share information when you request or authorize us to do so.

 

We do not knowingly sell personal data in exchange for money. If our practices change, we will update this policy and provide required notices and choices under Colorado law.

5) Colorado Privacy Rights

Colorado law (including the Colorado Privacy Act) may provide Colorado residents certain rights regarding their personal data, including:
 

  • Right to Access: confirm whether we process your personal data and access it

  • Right to Correction: correct inaccuracies in your personal data

  • Right to Deletion: request deletion of personal data we have collected about you

  • Right to Data Portability: obtain a copy of your data in a portable format (when applicable)

  • Right to Opt Out: opt out of certain processing, including:

    • Targeted advertising

    • Sale of personal data (as defined by law)

    • Profiling in furtherance of decisions that produce legal or similarly significant effects
       

How To Exercise Your Rights
 

To submit a request, contact us using one of the methods in Section 12.
We may need to verify your identity before completing your request. In some cases, we may request additional information to confirm you are the person (or authorized agent) making the request.

 

Appeals Process
 

If we deny your request, you may appeal our decision by contacting us at: [appeals email] with the subject line “Colorado Privacy Appeal.” We will respond within the timeframe required by applicable law.

6) Sensitive Data

Some information may be considered sensitive (for example, certain health-related details). When required, we will obtain consent before processing sensitive data, and we use safeguards appropriate to the sensitivity of the information.

7) Cookies And Tracking Technologies

We use cookies and similar technologies to:

  • Understand traffic and usage patterns

  • Help prevent fraud and improve security

You can control cookies through your browser settings. If you disable cookies, parts of the Site may not function properly.

8) Data Security

We maintain reasonable administrative, technical, and physical safeguards designed to protect personal data. However, no system is 100% secure, and we cannot guarantee absolute security.

9) Data Retention

We keep personal data only as long as reasonably necessary for the purposes described in this Policy, including to provide services, meet legal obligations, resolve disputes, and enforce agreements.

10) Health-Related Information And HIPAA

If Care Foundation Services is a covered entity or business associate under HIPAA, certain information you provide as part of services may be treated as Protected Health Information (PHI) and handled according to HIPAA and applicable state requirements. Where HIPAA applies, HIPAA may control over this Privacy Policy for PHI.

 

If you would like a HIPAA Notice of Privacy Practices (if applicable), contact us using the information in Section 12.

11) Children’s Privacy

Our Site is not intended for children under 18, and we do not knowingly collect personal information from children under 18 through the Site. If you believe a child has provided us personal information online, please contact us so we can take appropriate steps.
