Our Services
Our services are built around real-life daily routines, personal goals, and the environments where people feel most comfortable. Whether support is needed at home, out in the community, or while getting to and from meaningful activities, we tailor each service to match the individual’s preferences and level of independence. With steady guidance, respectful care, and a focus on safety and growth, we help create a supportive path that makes everyday living more confident, connected, and fulfilling.

Individual Residential Services and Supports (IRSS)
Our Individual Residential Services and Supports (IRSS) provide around-the-clock assistance for individuals with intellectual and developmental disabilities (IDD) who want to live as independently as possible in the community of their choice. IRSS supports small living arrangements—typically up to three individuals—within a Personal Care Alternative (PCA) setting or a Host Home setting. These living options are flexible and are designed around each person’s unique needs, preferences, and goals. Individuals may live in a home owned or leased by the agency, in their own home or apartment, or in a Host Home environment.
As part of IRSS, we take the lead in developing each person’s Individual Services and Supports Plan (ISSP) and serve as an active member of the person’s Interdisciplinary Team (IDT). We also provide ongoing quality monitoring, 24-hour on-call support, nursing support, medication administration (as applicable), special diet accommodations, accessible living options, transportation to medical appointments, access to community resources, and opportunities to build meaningful relationships. Individuals can choose the model and environment that best fits their needs, and if available options don’t align with their living choices, we support a smooth transition to another provider.

Community Connector (CC)
Community Connector (CC) services are one-to-one supports designed to help individuals build confidence, relationships, and natural supports in typical community settings. CC focuses on promoting safety and independence while helping the individual work toward personal goals through real-life experiences. Services can be provided as a one-time support or on a recurring schedule, depending on what the individual wants and what best supports their progress.
Through CC, individuals gain experience accessing their community competently and safely, while exploring cultural, social, recreational, volunteer, and future employment opportunities. We support individuals in planning and choosing activities, learning how to navigate community resources, and building skills that strengthen independence, decision-making, and comfort in public settings.

Supported Community Connections (SCC)
Supported Community Connections (SCC) helps individuals connect with their community through either one-to-one support or small groups of two to five people. SCC services are designed to encourage meaningful relationships, natural supports, and community participation while promoting safety, independence, and progress toward personal goals. Services can be provided one time or ongoing, based on the individual’s needs and preferences.
SCC supports individuals in building confidence while accessing community opportunities such as recreational activities, cultural events, volunteering, and employment exploration. Individuals are encouraged to plan, choose, and learn in real-world environments—developing practical skills, strengthening social connections, and increasing independence through supported experiences.

Homemaker Basic
Homemaker Basic services support individuals with intellectual and developmental disabilities who need assistance managing everyday household tasks so they can live as independently as possible in their home. Services are provided in the person’s residence and are focused on maintaining a clean, safe, and healthy living environment. Support may include completing tasks for the individual when needed, or providing cueing and prompts to help them complete tasks on their own depending on the goals in their Individual Plan and approvals from their Case Manager/Resource Coordinator.
Homemaker Basic may include support with laundry, cleaning and changing linens, folding and organizing clothing, dusting, vacuuming, mopping and floor care, general bathroom and kitchen cleaning, indoor window cleaning, trash removal, sorting and organizing, and light general upkeep tasks that help maintain a stable home environment.

Homemaker Enhanced
Homemaker Enhanced services include the same household supports as Homemaker Basic, with the added benefit of either habilitation support or extraordinary/deep cleaning provided by a qualified homemaker. This service is designed for individuals who need additional assistance to remain healthy and safe at home, especially when skills need to be developed or when the home requires a higher level of cleaning support.
Habilitation focuses on helping the individual build the skills needed to maintain their environment through step-by-step instruction and learning strategies tailored to their learning style. Extraordinary cleaning addresses deeper or more intensive cleaning needs that go beyond routine tasks. The level of support, training goals, or deep-cleaning areas are identified in the person’s Individual Plan and approved by their Case Manager/Resource Coordinator, ensuring services remain aligned with the person’s needs and outcomes.

Mentorship
Mentorship services are designed to help individuals living in the community develop strong self-advocacy skills and greater independence in decision-making. Mentorship is guided by the individual’s goals and can include learning to speak up for needs and preferences, building confidence in community environments, and practicing skills that support personal growth and leadership. Services are provided in the community at times and locations chosen by the individual.
Mentorship may include support with exploring community experiences, interviewing potential providers or generic support professionals, and gaining exposure to opportunities such as participation on private or public boards, advisory groups, or commissions. The goal is to help individuals become informed, confident self-advocates who can make choices, navigate systems, and build independence with pride.

Non-Medical Transportation
Non-Medical Transportation services provide safe, reliable transportation for individuals with intellectual and developmental disabilities so they can access meaningful day activities. This service supports individuals traveling to and from their home and approved destinations such as day programs or job settings, helping them remain active, engaged, and connected.
Our transportation support prioritizes safety, consistency, and dependability—ensuring individuals have a reliable way to attend programs, maintain routines, and participate in community life without unnecessary barriers.
